初めてお越しの方は、日本の保険証に加え、
在留カード・運転免許証・マイナンバーカードなど、
顔写真が記載された身分証明証をご提示いただきますよう、
お願い申し上げます。

We ask all patients visiting for the first time to show us a Japanese health insurance card and an ID card that has your picture on such as resident card, driver license and my number card.

※ If you don't have an ID card, our sincerest apologizes but you CANNOT see our doctor.
※ If you only have your ID card and you wouldn't mind paying all costs without being covered by Japanese health insurance, you can see our doctor on the day.
When you bring your insurance card next time you visit our clinic,
we can pay you back the difference.
We kindly ask for your understanding and cooperation.
Prescriptions for dispensing pharmacies will be issued for your medication in most cases. Please pick up your medicine at dispensing pharmacy located outside the hospital within four days (including the day of issuance)
